APPLY FOR A PATIENT AND FAMILY ASSISTANCE GRANT
Team Continuum is all about the here and now: the care, not the cure. We help people and families living with a cancer diagnosis today by providing immediate and vital assistance ? both practical and personal. We pay their household expenses. We offer friendship and moral support. We fund health facilities to enhance the delivery of patient care, communications and educational services. We do everything in our power (and bank account) to minimize the burdens of everyday life so they can focus less on stress and more on getting better.
GUIDELINES TO APPLY FOR A GRANT
Your application must be complete and in total compliance before it can be accepted for consideration.
Applications must be submitted by a licensed social worker on your behalf.An application submitted by any person other than a licensed social worker will not be reviewed by our staff.
Team Continuum now has a limited geographic reach. Applications will only be accepted from social workers affiliated with hospitals or treatment facilities located in the following states: NY, NJ, CT, MA, NH and RI.
Applications will be accepted between the 1st and 15th day of the month, as determined by the postmark if mailed or the date of the fax or email.
Applications received after the 15th day of the month or in advance of the 1st, whether by email, fax or postal mail, will be either discarded or returned.
Section 1 of the application must be completed by a licensed social worker only. This section must not be left blank.
Section 6 of the application must be completed by your Oncology MD or your Oncology RN.
Only applications containing ALL information, ALL bills, and ALL other required documentation will be considered. Do not submit original documents or bills. COPIES ONLY.
We require documentation of all family income, assets and bills. Review the Checklist in Section 7 to ensure that your application is complete. Please do not submit tax returns.
Applications with incomplete sections, and/or missing copies of bills and documents will be discarded or returned.
Do not send your application in parts and send only once. All sections and attachments must be included in one package. Chose mail, email or fax. If submitting by email or fax, send only once.
IF YOU RECEIVED ASSISTANCE IN THE PAST, PLEASE WAIT AT LEAST 6 MONTHS TO APPLY AGAIN
